Set up a help plan
Source: BUS Rheinland-PfalzRegardless of which form of "help for education" you apply for at the Youth Welfare Office - the first step is to create a help plan together with all those involved.
The help plan defines the cornerstones of the help:
- Need
- Facts from your family (educational situation, economic circumstances, caring for the child, physical and mental condition and the like)
- If you have already used other aid measures, what were they? Why may the help not have been successful?
- Your child's personal form (e.g. who are the parents, siblings, relatives, abnormalities, illnesses, allergies, vaccinations)
- Objective of aid measures
- What should change as a result of the aid?
- What expectations do those involved have?
- Type of help
- Which services are considered necessary and suitable (e.g. full-time care, educational assistance, socio-educational family assistance)?
- Proposals for action for parents, professionals, caregivers
- Time horizon (How long should the aid last?)
- necessary services (e.g. care allowance, initial equipment, therapy costs, travel expenses, financial support)
The Youth Welfare Office must inform all those involved about possible consequences of the planned help for the child or young person
