If your veterinary license certificate has been destroyed or lost, a duplicate can be issued to you. With the issuance of the duplicate, your original document will be declared invalid, if it is found again later, you must return it.

Certificate of good conduct of document type "O" stating the purpose of use "Duplicate of my approbation/professional permit". This must be applied for at the registration office of your place of residence and sent directly to the State Investigation Office.
Submission of an insurance that the original of my professional license has been lost.
Voluntary commitment to send the original as well as all copies, duplicates or certified copies to the Landesuntersuchungsamt immediately if they are found.
Make the following declaration:
"I have taken note of the fact that the professional license loses its validity through the issuance of a duplicate. In accordance with § 52 VwVfG, I am obliged to surrender the original document (if available or recovered)."
