Return approbation

The licence to practise as a veterinarian may be waived by written declaration to the competent authority.

With the renunciation of the license to practice medicine, both the right to practice the profession and the right to use the respective professional title are abandoned.

Written, unconditional declaration that you waive your license to practice medicine as a veterinarian
Notification of where you last exercised the profession as a veterinarian and from when the waiver should claim validity
Details of the licence certificate (place of issue, issuing authority and date of issue of the certificate; Name to which the certificate is issued)
